Behind every transformation is a story worth celebrating. This year, we launch the Transition Impact Awards, presented by AVI Human Services, proudly recognizing states and agencies that exemplify excellence in service delivery through our Transition Readiness Toolkit. These awards celebrate organizations and individuals whose dedication results in measurable progress, consistent engagement, and transformative outcomes.

Spanning the data from August 1 to July 31 each year, the Transition Impact Awards highlight those who have driven measurable, meaningful change across service areas. These agencies guide participants who are often just beginning their journey—some starting with the lowest levels of readiness—and equip them with the tools, encouragement, and structure they need to grow. Through consistent engagement, compassionate coaching, and innovative programming, these providers are making a lasting difference.

These awards are not about the Transition Readiness Toolkit or the systems behind it; they are about the transformations themselves. The real heroes of these awards are the young people whose stories are written in the upward curves of each chart.
